The core information
In 2018/19 we began work on a new project to support better integrated care through digital shared care records. Working with NHS England, the PRSB has asked citizens and health and care professionals to help produce a ‘core information standard’ that defines exactly what information should be shared in a person’s care record throughout their life.
Nearly 1500 patients and professionals shared their views on what information is needed to support good care in any setting. The standard will be piloted in five regions in England, covering 24 million people, but is designed to be used across the whole of the UK.

The standard will support better integrated care, by ensuring that all services have access to the same information about a person’s care. By working collaboratively, we will be able to make better clinical decisions and support people to stay in control of their own health and care and avoid hospital visits where possible.”
