Pharmacy information flows
As the number of people living with long-term conditions continues to grow, pharmacies are providing more services in the community than ever before. In 2018/19, we launched a new pharmacy standard to outline the information that needs to be shared between pharmacies and GP practices.
Once used, it will support a reduction in medication errors, improve patient safety and reduce administrative work for busy staff.
The information shared will include emergency prescriptions, vaccinations, medication and appliance reviews, as well as medications advice for minor illnesses. By making this accessible to the right professionals, it will support better integrated care in the community.
